Cognitive Symptoms in Type 1 Diabetes Patients.

Clinically significant cognitive impairment is present in more than a quarter of middle-aged adults who had been diagnosed with type 1 diabetes in childhood, according to a new study.

The results were published online July 7, 2015 in Diabetes Care by Karen A Nunley, PhD, a postdoctoral associate in epidemiology at the University of Pittsburgh, Pennsylvania, and colleagues.

Wide neuropsychiatric testing among 97 adults with an average age of 49 years who had been diagnosed with type 1 diabetes in childhood revealed a prevalence of 28% of clinically significant cognitive impairment, which is comparable to the prevalence of mild cognitive impairment typically reported in adults aged 85 years and older.

The finding raises several concerns, Dr. Nunley told Medscape Medical News.

"We are studying adults who were diagnosed mainly in children, whereas previous studies included individuals diagnosed in adulthood, people who were not exposed to metabolic dysregulation associated with diabetes during critical periods of brain development that occurs during the childhood. "

And, he said, the issues are also middle aged adults "who are at risk of changes" normal "age-related cognitive, the effects of which may be amplified by long-term exposure to diabetes."

One explanation could be that chronic hyperglycemia has negative effects on the cerebral vasculature, similar to that known to occur in the retina or the kidney, he said.

And one of the main concerns is that cognitive impairment can affect a patient's ability to manage their own diabetes. The Poor executive function could hinder time management, the ability to plan and organize, remember details, and to change the approach, Dr. Nunley said, also adding that impaired psychomotor speed may affect the patient's ability to respond quickly to a dangerous situation - for example, while driving.

However, she also stressed that "we are in no way implying that those with onset type 1 diabetes in children can not become successful adults with significant careers. What we hope to do is raise awareness that cognitive impairment It is a possible complication of diabetes, along the lines of retinopathy and neuropathy, so medical providers can help people with type 1 diabetes and their caregivers understand why this happens. "

Today everybody in the world especially in India are talking about banning of Nestle Maggi and withdrawn of crores of Maggi stock from the market.

      As we know that till 2 decades Maggi became a part of our life. From 2 years children to 60 years old people eating Maggi as their daily food. It is very difficult to withdraw Maggi from the society because people are addicted to it.

Some reasons why people loving Maggi:
1. It is easy to cook.
2. It is tasty
3. Easy to digest
4. As children does not eat vegetables mothers make them to eat vegetables through Maggie.
5. Some body think it is healthy.
6. It is cheaper when compare to other food.

Government voice towards Maggi:
1. Maggie contains more lead than threshold value.
2. Long term consumption of Maggi leads to lead poisoning called as Lead palsy.
3. The symptoms of lead palsy are brain degeneration, body growth restriction etc.,

Facts about banning of Maggi :
1. it might be the government plan to withdraw Maggi from the market to counter the foreign economy. Because Maggie making more marketing in India.
2. Some body telling that it is plan of Maggie against companies.
3. If Maggi contains lead more than threshold value then why we don't find even one case because of lead poisoning.
4. It is very difficult to withdraw any product from the market when the market is at high range.
5. If maggi noodles contains lead what about other noodles. They are also using almost same formula as maggi noodles.

Alternative steps to decrease lead poisoning instead of banning Maggi:
1. All are telling that it is dangerous to children health then it is suggested that the label content of Maggi will be changed to NOT FOR CHILDREN and they should mention age group on label.
2. The effects of the packed content should be printed on label as we seen in case of tablets. Then the people who want to eat they can eat and who don't want avoid it.
3. Parents don't cook Maggie for children below 12 years to avoid lead poisoning.
4. The final important thing is that government has to take steps for research on Maggie poisoning to prove its harmful contents. Then it will be helpful for public and its employees.

Be Alert of Sun Stroke

Sun Stroke:
        It is defined as the stroke of heart brought about by excessive heat of sun. It is very common in tropical areas like India at summer season. This is common at summer season i.e,. April to June. This condition may be mild to severe. Some times it is fatal especially for pediatrics and geriatrics.

         So, every one should have knowledge about sun stroke to get rid of its fatal effects. Here I am trying to give little idea about symptoms, causes and first aid of sun stroke.

Causes of Sun stroke:
1. Over working in sun heat (especially between 12 Pm to 4 Pm)
2. Fasting in summer seasons.
3. Lack of water drinking

Symptoms of sun stroke:
1. Over thirsty.
2. Giddiness and fatigue.
3. Palpitation.
4. Dry mouth and dry lips.
5. Fever and diarrhea.
6. Hypotension.

Effects of sun stroke:
1. The common effects are weakness.
2. Some times the effects are fatal like excessive diarrhea.
3. Sudden heart stoke.

How to detect Sun stroke:
If any of the above symptoms were seen in patient we decided that it is sun stroke. The patient looses electrolytes due to sun heat so the blood pressure decreases it may leads to decreasing of blood supply to heart which results heart stroke.

First aid for sun stroke:
1. Patient has brought to cool place immediately.
2. Loosen the body clothes to encourage breathing.
3. Give plenty of water.
4. If possible give ORS.
5. If symptoms are severe take patient to hospital immediately.

1. Drink plenty of water.
2. Use umbrellas and caps while going to out side in between 12 Pm to 4 Pm.
3. Take fruit juices, coconut water and fruit juices.
4. Wear cotton clothes.
5. Take rest.

Zoledronic acid

     Zoledronic acid is the drug used to treat Osteoporosis and other bone disorders especially for women. It is having several side effects like Osteonecrosis of the jaw.

     Recently, European union advisory board has recommended measures to minimize osteonecrosis of the jaw. Pharmacovigilance Risk assessment committee(PRAC) of Europe has advised that the patients has to remind about adverse effects of Zoledronic acid and patients has to provide leaflets about side effects of drug.

       PRAC also advised that patients has to provide leaflets which contain advantages of treatment with Zoledronic acid and disadvantages of treatment as osteonecrosis of jaw. It is very common side effect in menopause woman than men.

Statins-Contraindicated for type 2 Diabetes

Statins:

         Statins (or HMG-CoA reductase inhibitors) are a class of drugs used to lower cholesterol levels by inhibiting the enzyme HMG-CoA reductase, which plays a central role in the production of cholesterol in the liver, which produces about 70 percent of total cholesterol in the body. High cholesterol levels have been associated with cardiovascular disease (CVD). Statins have been found to prevent cardiovascular disease and mortality in those who are at high risk. The evidence is strong that statins are effective for treating CVD in the early stages of a disease (secondary prevention) and in those at elevated risk but without CVD (primary prevention). Side effects of statins include muscle pain, increased risk of diabetes mellitus and abnormalities in liver enzyme tests. Additionally, they have rare but severe adverse effects, particularly muscle damage.

Type II DM is the disease in which insulin deficiency occurs though insulin production is normal but the uptake of insulin by tissues may disturbs leads to imbalanced blood sugar levels. This disorder will be treated with oral hypoglycemics.

Statins are contraindicated for Type II DM patients:
Recent studies telling that usage of statins (HMG-Co A Reductase inhibitors) in CVD patients who have the history of Type II DM may leads to several health disasters as sudden death and sometimes leads to severe heart diseases. Apple has seeded iOS 8.2 beta for multiple developers, the SDK WatchKit and some other bug fixes. iOS 8.2 could be released in March. iOS 8.3, which adds features like wireless CarPlay and a new emoji selector is also in development.

Panobinostat - Get Approval by FDA for the treatment of Relapsed Multiple Myeloma

        The US FDA recently gave approval for Panobinostat for the treatment of Relapsed Multiple Myeloma. Previously in 2014 it was rejected by FDA Oncologic Drugs Advisory Committee  due to its adverse effects. After rejection the manufacturers again submitted additional information to FDA about drug.

      By studying all the information FDA approves Panobinostat.  It is approved to use along with bortezomib and dexamethasone. It is effective in the treatment of Relapsed multiple myeloma in patients who received bortezomab for past 2 years. It is the new class of agents that acts as histone deacetylase inhibitor.

Mechanism of Action of Panobinostat


       It is having new mechanism of action than other cancer agents as histone deacetylase inhibition makes this drug attractive among other cancer drugs. The safety and efficacy data of Panobinostat is based on the clinical study on 193 patients who had received prior treatment with bortezomib and immuno modulatory agents. The clinical phase used is phase 3, randomized,double blind,placebo controlled, multicenter global registration trail. This drug is having several adverse effects which make it rejection by FDA previously.

Adverse effects:
The common adverse effects are,
1. Nausea
2. Vomiting
3. Diarrhoea and
4. Fatigue.

        The most common side effects of this drug in clinical trails are diarrhea, fatigue, nausea, peripheral edema, decreased appetite, pyrexia, and vomiting, with the most common nonhematologic laboratory abnormalities.

Panobinostat capsules


         Common hematologic abnormalities include thrombocytopenia, lymphopenia, leukopenia,neutropenia and anemia. The serious side effect include cardiac toxicity and severe diarrhoea.

So, the patients who receiving panobinostat had to monitor for the adverse effects especially for anemia.

Ovarian cancer risk may increased by Hormone Replacement Therapy

         
           At menopause stage the women bodies stop producing sex hormones (estrogen,progesterone) which are important to maintain normal physiological functions like metabolism,mental activities etc., So after menopause (at post menopausal stage) woman starts to take Estrogen and progesterone pills under the super vision of gynecologist. This is called as Hormone Replacement Therapy (HRT) .

     HRT therapy having some side effects. These are common and mild in nature like dizziness,weight gain,mood swings etc., Recent studies reveal that long term use of HRT may increase the risk of ovarian cancer. The researchers conducted epidemiological studies at U.S by using case and control group and concluded that HRT may increases cancer risk.

            It is very severe in the patients in whom ovaries are dissected at the time of hysterectomy. But risk is lesser in patients with ovaries. The researchers conduct case control study in which, 
1.case group is Patients are post menopausal and having ovarian cancer
2. Control group is patients are postmenopausal but not having ovarian cancer and still having their ovaries.

            They compare both groups by conducting meta-analysis and concluded that the usage of HRT for even less period may increases the cancer risk up to 20% . The researchers also told that patient related factors like body weight,hysterectomy, other diseases does not shows effect on HRT. The more the recent the therapy the great the risk. Woman who had stopped HRT recently but were within 5 years of last use at the time of their cancer risk , although having used HRT for at least 5 years was still associated with 10% increased relative risk more than 5 years later.

New FDA Clearance for Ranibizumab for Diabetic Retinopathy with DME

    
     The USFDA has expanded sate use of Ranibizumab for diabetic retinopathy ane Diabetic Macular Edema (DME) treatment. Diabetes is one of the deadly disease that affecting millions of people every year world wide. In this disease the sugar levels of blood will changes time to time with our order. The patient looses body sugars in urine which makes fall down of sugar levels.


    At the time of eating body sugar levels will increases suddenly which damages body organs. Diabetic patients are more prone to organ damages. The main organ is eye. Several diabetic patients are suffering with diabetic retinopathy which impairs vision. So it needs proper treatment. Organ failure chances will increase with age. It is the most common diabetic eye disease and is leading cause of blindness in adults in US.

    Previously FDA had approved ranibizumab to treat DME and macular edema secondary to retinal vein occlusions, both of which cause fluid to leak into the macula result in blurred vision. Recently FDA granted the Ranibizumab for diabetic retinopathy with DME and reviewed this new use for the drug under the agency's priority review program, which provides for an expedited review of drugs. This drug provides safe and effective treatment in serious condition.

Route of administration:
Ranibizumab is administered by IV injection once monthly.



Other uses of Ranibizumab:
It is also approved for the treatment of neovascular (wet) age related macular degeneratin in US.

The recent studies conducted in U.K telling that cancer deaths can be prevented in the age group of below 80 years by the year of 2050. Cancer occurrence at younger stage and adult age below 80 tears can be cured by using present treatment methods.

      This is the question that everyone will ask after hearing this news. Here, is the answer. As we know that the main problem in the treatment of cancer is, delay in the detection. NEW OBESITY TREATMENT GUIDELINES ACCORDING TO AHA

NSAIDs protects from skin cancer

What are NSAIDs:
Non Steroidal anti Inflammatory Drugs are the agents which are having 3 types of activities
a. Anti pyretic
b. Anti inflammatory and
c. Anti analgesic activity

Significance of NSAIDs:

Side effects of NSAIDs:
1. Hepatic failure at higher doses for a longer period of time
2. Gastric irritation, gastric bleeding

Contraindications of NSAIDs:
1. NSAIDs are contraindicated for renal impaired patients because NSAIDs are excreted through renal route.
2. NSAIDs are contraindicated for hepatic impaired patients because it is metabolized by glucuronidyl pathway of liver.
3. Contraindicated for gastritis patient as NSAIDs causes gastric bleeding.

Now it is evident that NSAIDs also protects from Squamous Cell Carcinoma (SCC), one type of skin cancer. NSAIDs like Aspirin and Non Aspirin usage protects from SSC AND other cancers. 

Mechanism by which NSAIDs protects from SSC:
NSAIDs inhibits Cyclooxigenase (COX-2) enzyme and inhibits prostaglandins (PG's) which are the mediators of inflammation. So, they inhibit the formation of tumors. By this they protects from SCC. But NSAIDs will not protective against all types of cancers but can protect from some types of tumors as we discuss above. 

CELL PHONES DID NOT CAUSE BRAIN CANCER (Brain Tumor)

        This is the new research that counters the earlier evidences which told that usage of mobile and cordless phones causes cancer. Several researchers told that cell phone usage causes health disorders like skin cancers, brain tumors.
      One research that conducted in U.S told that it causes "Gliomas", type of brain tumor. To counteract this one other group of researchers starts study on people who are frequently using mobiles. It is not easy to conduct Randomized control trail. Because RCT needs to follow group of patients to a certain period of time and also it needs large test population.
        It is also very expensive to conduct. So they conduct case control and cohort study on 1,00,000 people over 10 years. They conduct study in 5 countries all over the world. They observe that the incidence of cancer is only to small extent i.e., 9 in 10,000 people. It is not that much severe condition. So, researchers telling that cell phone usage do not cause brain tumors.
But we have to mind that the radiation releases from mobiles and cordless phones are very dangerous. They can affect health of pregnant women. They also dangerous to children aged below 10 years.
Effects of radiation:
1. It affects pregnant health condition.
2. Affects children health especially brains.
3. Affects bird’s health.
4. May causes some types of skin cancers.
        So, we have to cautious about mobile usage. Especially pregnant women must use head phones while talking in mobiles. Parents never allow children(less than 10 years) to use mobiles more than 1 hour. Every one should use radition free mobiles to save ourselves from radiation.

Anti Microbial Resistance (AMR):
     
    This is the condition in which the microbe shows resistance to the respected antibiotics. It mean that the microbe will not killed by antibiotic. This is also called as antibiotic failure.
Eg: Multi Drug Resistance by bacteria Mycobacterium Tuberculae.
     
      Antibiotic resistance is the alarming future disaster in all over the world. Antibiotic is one of the chemical compounds that are developed to counteract bacteria, virus and all other microbes.
   
     They counteract microbes and kill them to protect body. Now a day’s means past from 20 years microbial infections occupies major part in disease conditions. Simultaneously antibiotic use also increases. Physicians are prescribing more antibiotics even for small diseases. Some physicians are prescribing strong antibiotics. So the antibiotic resistance is developing in the present society drastically.
      
     Once one microbe will develops resistance it never be killed by any antibiotic. So physicians should mind about this while prescribing antibiotics. Any treatment for infections by antibiotics must start with weak antibiotics. If that antibiotic dies not show action one should go for another antibiotic. But never use strong antibiotic initially. Antibiotic prescription needs evidence of culture sensitivity test. Prescribing antibiotic without culture test is not a rational prescribing.

      Researchers and professionals telling that antibiotic resistance will be the upcoming future disaster that the world should face.  It is the severe disaster than all other diseases. So, we have to search ways to avoid this.
Till today 7,00,000 people are suffering from AMR (Anti Microbial Resistance).
By the year 2050 it may be 10 million.
